Issue Symptoms:

The Symptoms of this issue occurs when you go to Design->Monthly Data Entry Design. You are presented with the error. When you push ok, you are presented with a normal design screen except the Form Name is txtFormName. You are able to add new MDE forms and edit them but pushing any button on the form gives you this error.

This issue generally happens right after upgrading from OPS32.

Solution:

This error is a little misleading. The Record Set that it is looking at is not the Monthly Data Entry Records. It's actually looking at the Location Records. To fix the error, simply add a record under System Setup-> Location Setup.
