Saves the current form.   You will be prompted to name the report and pick a report group

FIELDS:

NAME: The name for the form. 

ENTRY FORM GROUP:  The Entry Group this form should be assigned to.

BUTTONS:

	Saves the Report
	Cancels the save and returns to the designer.
	Allows you to create a new entry group.
